Three community coalitions are recent recipients of a grant to create new and existing partnerships to lessen the impact of diabetes in high-risk communities. Coalitions in Elizabeth City, NC, Columbia, SC and Sumter, SC have all received grants from the Racial and Ethical Approaches to Community Health: South-Eastern African American Center of Excellence to Eliminate Disparities, (REACH US: SEA-CEED), based at the Medical University of South Carolina College of Nursing. Their goal is to empower communities to identify and implement prevention and control.
